To be eligible for referral to the Kidney Society, patients must meet the following criteria:
- Chronic Kidney Disease (CKD) Stages 4 or 5.
- Must reside in Hawke’s Bay, Tairāwhiti, Lakes, Bay of Plenty, Waikato, Auckland, or Northland.
- Must be a New Zealand resident.
- The patient must have given consent for the referral.
Who can refer?
- Healthcare professionals (GPs, renal specialists, nurses, allied health professionals, etc.) Patients can also self-refer.
